The following forms are available in TigerWeb, in the section "Student Forms":

• Application for In-State Tuition
   
• Personal Information Update
   
• Intent to Graduate Forms All students (TCAT & Credit) must submit an Intent to Graduate Form for the term they are completing all coursework or clock hours. Submission will initiate the commencement ceremony invitation, posting your credential on your transcript and printing your diploma/certificate.
   
• Change of Major or Program
   
• Authorization to Disclose Educational Records

Certification of Total Disability
This form is required for a student with total disability who is seeking a reduction of fees.

Out-of-State Resident Employment Verification
Part-time out of state students may be eligible for an in state fee reduction if they work full time in the state of Tennessee.

Enrollment Verification & Transcript Request
Request Enrollment Verification or a Transcript Request through the National Student Clearinghouse.
